Our Story

Transform A Nation, Inc. (TAN), is a community-based mental health organization that customizes client services based on their needs.  We are a 501 (c)(3) and we are CARF-accredited.  Initially licensed as a Psychiatric Rehabilitation Program in 2019, we dedicated our time and services to addressing the core of our clients’ challenges and walking with them as they begin and engaged in their personal journey of mental health stability. Our intensive and comprehensive approach to rehabilitation care continues to help individual’s work through the challenges of their crisis to achieve full recovery and reintegration into the community.

 

In May of 2022 our Executive Director thought it imperative to expand our services so we could increase our reach and presence in the community, and our Outpatient Mental Health Program and Supportive Employment Program was formed.  We address the emotional, behavioral, vocational, and mental health needs of youth and adults.  Our licensed clinicians and support staff apply public health principles in our delivery of mental health services to help our clients achieve stability and thrive in their families and communities. We understand that the needs of each of our clients are unique and we create individualized treatment plans in collaboration with our clients.  Treatment plans include both one-on-one meetings and may include group sessions as a component of the therapeutic approach we engage. Our desire is to services families, from a holistic perspective, as we believe uplifting the entire family will result in more positive and favorable outcomes for our communities.
